*To be eligible to be on POWER IV : FORCE, the production requires that you verify that you are “Up To Date” (FULLY VACCINATED + BOOSTER SHOT COMPLETED if eligible for) on your COVID-19 vaccination. PLEASE ONLY SUBMIT YOURSELF IF YOU ARE FULLY VACCINATED AND WILLING TO PROVIDE DOCUMENTATION OF YOUR VACCINATION STATUS TO THEIR PROJECT’S HEALTH & SAFETY DEPARTMENT.
“Up-to-date” means the following:
-A two-dose Pfizer COVID-19 vaccination series, and if eligible, a booster dose for those who are 5+ months past their second shot. Currently, the Pfizer booster is approved for everyone ages 12+.
-A two-dose Moderna COVID-19 vaccination series and if eligible, a booster dose for those who are 5+ months past their second shot. Currently, the Moderna booster is approved for everyone ages 18 and up.
-A single-dose Johnson & Johnson COVID-19 vaccination, and if eligible, a booster dose for those who are 2+ months past their single dose. Currently, the Johnson & Johnson booster is approved for everyone ages 18 and up, but an mRNA booster is recommended in many cases.
-OR If you are not yet eligible for your booster shot, your vaccine status is considered up to date.  You must obtain your booster promptly once you become eligible and submit validation that you received your booster dose.

Send an email to extras4force@gmail.com with “HOT DANCER” in the subject line:
1) Confirm that you have a POP profile.
2) Include Name, Phone, Email Address, and a current photo
3) Confirm that you are generally available for filming
4) Confirm you are “UP TO DATE” (Vaccine Shot(s) + Booster Shot (If eligible)) and are willing to provide documentation of your vaccination status to Health & Safety (DO NOT SEND YOUR ACTUAL VACCINATION CARDS TO 4 STAR TEAM).
5) List your current sizes: Jacket, shirt (neck/sleeve), pants waist, head size and shoes. Please list your height and weight as well, and update these in POP if they are not currently accurate or listed.
6)Please confirm you can dance

If selected, they will send you an EMAIL to submit your UP-TO-DATE VACCINATION STATUS (Vaccine Shot(s) + Booster Shot (If Eligible)) and once cleared by the Productions Health & Safety Department, you will receive a detailed availability check and could receive a booking email all through our POP platform.
